Score ranges

Fragile

1.0 – 2.4

This dimension is a real constraint on the team. Address it before layering on new process.

Developing

2.5 – 3.7

Present but inconsistent. Look for which behaviours happen only sometimes.

Healthy

3.8 – 5.0

A working strength. Name the specific habits producing it so they survive team changes.

How to score and run it

  • Use it anonymously — psychological safety items are the first to be distorted when responses are identifiable.
  • Reverse-score the four negatively worded items, then take the mean per subscale.
  • Compare the same team over time rather than comparing different teams to each other.

This is a self-report questionnaire template for feedback and reflection. It is not a clinical or diagnostic instrument, and scores should not be used to make decisions about an individual on their own.

Run this with automatic scoring

Psych Lab handles the reverse scoring, subscale bands, join codes and CSV export for you — and you can list the finished questionnaire publicly.
