Fragile
1.0 – 2.4This dimension is a real constraint on the team. Address it before layering on new process.
Developing
2.5 – 3.7Present but inconsistent. Look for which behaviours happen only sometimes.
Healthy
3.8 – 5.0A working strength. Name the specific habits producing it so they survive team changes.
How to score and run it
- Use it anonymously — psychological safety items are the first to be distorted when responses are identifiable.
- Reverse-score the four negatively worded items, then take the mean per subscale.
- Compare the same team over time rather than comparing different teams to each other.
This is a self-report questionnaire template for feedback and reflection. It is not a clinical or diagnostic instrument, and scores should not be used to make decisions about an individual on their own.
